How to Report a Claim
All claims should be reported promptly to your agent/broker and subsequently to ComStar General Insurance Solutions in accordance with your policy using the general claim forms available below or from your Agent/Broker.
When reporting a claim, please be sure to include the policy number and the name of the Named Insured as it appears on the policy Declarations Page.
New claims can be reported to ComStar General Insurance Solutions by sending the completed claim form to ComStar General Insurance Solutions using one of the following methods:
